Grant Agreement
This grant agreement template informs grantees, potential applicants and the general public of the standard terms and conditions under which grantees receive LSDF funding. Two template versions are included, one for grantee organizations that are state agencies and one for those that are not.
Specific details regarding the grantee organization and the grantee's project or program are included in the final grant agreements negotiated with the grantee organizations.

Sample Intellectual Property Agreement
Research and development activities between the grantee and subcontracting or collaborating organizations must be supported by an agreement that, at a minimum, makes explicit provision for the disposition of intellectual property rights among the organizations. If the grantee does not have a standard form for this type of agreement, the attached example can serve as an appropriate agreement for this purpose.

Milestones and Timelines Construction
Grantees are required to provide milestones, and the timeline for their achievement, with their grant agreement, and this document suggests an acceptable format and style.
Budget/Fiscal Documents and Procedures
Provided to assist grantees in understanding the management of LSDF grant budgets.

Reimbursement Requests/Invoice Submission
Grantees are required to submit invoices electronically to LSDF for reimbursement of grant costs.

Budget Revisions, Unexpended Funds, Extension, Surplus and Supplement Requests
Grantees are required to obtain pre-approval for budget revisions, expenditure of unexpended funds within a budget year, no-cost extensions, expenditure of surpluses remaining at the end of the grant, and requests for supplemental funds.

Annual Financial Report (AFR)
Grantees must submit AFRs to summarize annual expenditures of LSDF funds.

Provided to assist grantees in understanding LSDF's requirements for submission of progress and invention reports.

Semi-Annual Progress Report
Progress reports are due every six months of the grant term and are submitted online.

Invention Report
When an invention is made under LSDF funding, grantee's technology transfer office or equivalent must submit an invention report to LSDF and provide updates every six months.
